Review all the tabs location in this section. You will see information regarding ALL necessary permission forms for youth, adult, and chaperone participants for your group. 

The mailing of your group packet should include: 

• Participant Roster - This form helps you keep track of permission forms collected.
• Safe Environment Compliance Form (All adults/chaperones names will be listed on this sheet)
• Youth Participant Permission Forms for each participant
• Adult/Chaperone Permission Forms for each participant
• Housing List (Optional but highly encouraged)
  • Please note that all housing is gender specific. Males and females will have separate housing accommodations. Adults and Clergy serving as chaperones will be housed in the same building as minors.  

For each youth participant, you will need their parent/guardian to fill out the forms listed below. After you've collected their filled out forms, double check that all applicable fields are correctly filled out. 

• Student and Youth Activity Permission Form
• Medication Authorization and Permission Form (if taking medication)
• Parent/Guardian Release for Student or Minor (Noncommercial)
• Code of Conduct

All adults who are participating must follow the policies for safe environment guidelines. The group contact will be responsible for confirming that each adult participating is in compliance with the safe environment policies. In addition, the following forms must be filled out and returned. Section 9.3.6 – Parish Volunteers

**Any chaperone who is driving must be 25+. Any adult who will only chaperone must be 21+. Adults between the ages of 18 – 20 will be registered as Adult participants.

Permission forms that all Adults must complete: 

• Adult Consent and Release Form
• Code of Conduct

In the case that your parish or school is not attending and you would like to volunteer, please fill out the Volunteer Inquiry Form. To volunteer, you must be 21+.

Please note that all housing is gender specific. Males and females will have separate housing accommodations. Adults and Clergy serving as chaperones will be housed in the same building as minors. Siblings can be in the same room so long as it is requested.
